This feasibility survey collects information you choose to provide, including your name, city, state, ZIP code, email address, optional mobile number, communication preferences and survey responses.
The information is intended to evaluate interest in a possible December 27 Chicago event, understand participant geography and preferences, and contact respondents who have expressly chosen to receive project updates.
Contact permissions are optional and may be withdrawn. Phone numbers are not required unless you request text-message updates. No payment information is collected through this survey.
The organizer should limit access to the response database, use reasonable security measures, and should not publicly identify individual respondents without permission.
